Medical Referral

• A referral can come in the form of a Chronic Disease Management Plan (CDM) from a GP.
• A referral can be used for 5 maximum visits per year.
• Please check the referral is adressed to All Needs Podiatry – PO Box 5047 Alberton SA 5014 and in date.

Support at Home Program

• A referral from the company managing the package, along with a doctor’s referral, must be provided.
• Invoicing details and the contact information of the case manager overseeing the client’s care must be supplied.

NDIS

National Disability Insurance Scheme

• We accept NDIS participants who are either self-managed or plan-managed.
• To ensure a smooth processing, we request the following information:
  • Name and contact details of your support coordinator.
  • Invoiving email address for plan-managed clients.
  • Company name managing your NDIS funds.

DVA

Department of Veteran Affairs

• To access services under DVA, a valid D904 referral from your GP must be provided prior to or at your first appointment.
• This referral covers up to 12 podiatry visits and must be renewed upon completion of the cycle.smooth processing, we request the following information:

Private Payment

Private Health Insurance is Accepted

• Home visit podiatry services are eligible for rebates through private health insurance.
• Most major Australian health funds accepted (subject to individual policy cover).
• Clients must have podiatry extras cover to claim a rebate.
